Please bring your insurance information, referral request, and/or insurance authorization (if applicable) to your appointment.

MAMMOGRAM:
If this is your first time being screened with us, bring any past mammogram images. Don’t apply deodorant or powder before your mammogram since metallic particles from these products could show up your mammogram and create confusion. About 1 hour before your mammogram, take an over-the-counter pain medication to ease any possible discomfort from the exam.
